Every baby born in Ontario needs to be registered with the province. The Notice of Live Birth form, is completed by qualified medical staff and submitted to ServiceOntario and the Statement of Live Birth form, is completed by parents of newborns. The completion of these two forms creates a birth registration.
You can register a birth online, in the first 12 months after a baby is born.

The Office of the Registrar General has records for births in Ontario over the past 95 years. For older records:
